When providing value-based, high-quality, and timely post-acute care solutions in the home, healthcare providers and networks have historically left patients unsatisfied and still in need. Whether it’s the patient experience or foundational service delivery obstacles, there persists a stigma surrounding the reliability of care in the home.

2023 was a big year for tango! As we enter 2024, we wanted to take a moment to reflect on everything we accomplished in 2023. The past year was filled with building new capabilities and partnerships that will support us in our growth in 2024. Check out some of our key milestones.

Changes at tango

A significant marker of 2023 was our re-brand from PHCN tango. As our CEO, Brian Lobley, recently stated, “We wanted to usher in a new chapter of the company… We wanted a brand that signified partnership in the market. We wanted a brand that was memorable. It takes two to tango, and we’re firm believers that healthcare is a team sport.” We also grew our team to over 200 employees and expanded our leadership team – the driving force behind our mission of enabling quality care.

Clients Served

In 2023 over 1.1 million members were served across two business lines, ensuring they had access to home health care and received the best care possible with our highest quality providers. These members spanned across four geographies.

Key Outcomes

Additionally, we established strong provider partnerships with several national providers and created defining pilots for collaborating on care coordination efforts. We also built and launched our very first care coordination program, ensuring members received timely access to quality care.

Outcomes that we are especially proud of include:
  • 10.1% 30-day readmission rate*
  • 2-day reduction in start of care times**
  • 100% placement of all qualified referrals, including those in difficult to staff areas***

Looking ahead, we are excited about what 2024 will bring... big changes, new services, new partners, new markets, and most importantly, continuing to help our members get access to the best possible home health care.
